Foundations Nashville’s primary focus is to offer integrated treatment for co-occurring disorders. Oftentimes those dealing with addiction issues also struggle with co-occurring mental health issues like depression, anxiety, trauma or bipolar disorder.
After the completion of our Intensive Outpatient Program, you may be ready to step down to a more limited schedule of treatment that still provides some structure and support for their ongoing sobriety. Foundations Nashville offers aftercare sessions that focus on recovery planning, helping you to maintain your sobriety and stay focused on being well.

Our holistic therapy options are designed to inspire healing for the mind, body and spirit. Pairing clinical and psychological treatment with our intensive outpatient programs and holistic offerings, such as music therapy and yoga, helps you reconnect with yourself. We can also offer support for those who need help dealing with grief and loss, as well as anger management.

Intervention Assistance
We work with a core group of professional interventionists who practice a variety of models. Many misconceptions about interventions exist, but the emotional scenes you see on reality shows bear little resemblance to the average intervention. The primary goal of this process is simply breaking down barriers to treatment and getting the addicted individual help that’s needed. If you connect with us at Foundations Nashville, we can help you find an interventionist who may be able to break down your loved one’s barriers to treatment.
Aftercare Monitoring Program
If you’ve completed our program and need more support between IOP and a weekly outpatient therapist, we provide the Aftercare Monitoring Program (AMP) which provides additional support in the recovery process on a month-to-month basis. During this period, you will have access to therapeutic groups one to two times per week, a weekly aftercare support group, weekly yoga group and individual therapy and psychiatric appointments as you continue to build and engage in your community-based support system.
Alumni Services
Upon completion of your outpatient care and the recovery planning program, you can become a member of our alumni network. The goal of this valuable program is to aid you in maintaining a link with the people that you come to know during treatment and to provide continued support even after treatment has ended.
